We should avoid the use of Mafutsuto in conditions of extreme heat or cold, because complications, such as suppuration of surgical wounds, tend to occur more frequently under those conditions, leading to treatment failure. When giving surgical treatments, Gendai Kamata, our teacher, rather frequently administers Mafutsuto. For such patients, we perform six peri-anesthesia diagnoses, of which three are performed before the administration of the Mafutsuto, and three after the administration. Performing those diagnoses in detail is the most important process in the administration of Mafutsuto.<\/p>\n<p>Pre-administration diagnoses are made to assess whether the use of Mafutsuto is appropriate for a patient, and there are three major conditions for which the use of Mafutsuto is contraindicated; (1) if a patient is frail with pale face and wasted limbs, frequenly feverish and having low appetite, or not showing rise and fall of ki of the yingyang (ki means spirit),<\/p>\n<div class=\"clearfix divider\"><\/div>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-medium wp-image-185\" src=\"http:\/\/www.m.ehime-u.ac.jp\/school\/anes\/anes_wp\/wp-content\/uploads\/2016\/03\/mafutu12-197x300.jpg\" alt=\"mafutu12\" width=\"197\" height=\"300\" \/>(2)\u51fa\u8840\u5f8c\u5143\u6c17\u3092\u56de\u5fa9\u305b\u305a\u3001\u3042\u308b\u3044\u306f\u5fc3\u7aa9\u90e8\u304c\u3064\u304b\u3048\u308b\u611f\u3058\u304c\u3057\u3066\u3001\u75f0\u3084\u54b3\u304c\u51fa\u3066\u3001\u52d5\u4f5c\u3067\u606f\u306e\u3042\u304c\u308b\u8005\u306b\u306f\u6295\u4e0e\u3057\u3066\u306f\u306a\u3089\u306a\u3044\u3002(3)\u5fc3\u81d3\u306e\u52d5\u60b8\u304c\u5f37\u304f\u3001\u80c3\u304c\u3064\u304b\u3048\u3066\u5614\u5410\u3057\u3001\u3042\u304f\u3073\u3084\u80f8\u3084\u3051\u306e\u3042\u308b\u3082\u306e\u306b\u306f\u6295\u4e0e\u3057\u3066\u306f\u306a\u3089\u306a\u3044\u3002\u3053\u308c\u3089\u306e\u4e09\u3064\u306e\u72b6\u614b\u3092\u719f\u8003\u3057\u305f\u305d\u306e\u5f8c\u306b\u3000\u9ebb\u6cb8\u6e6f\u3092\u6295\u4e0e\u3059\u3079\u304d\u3067\u3059\u3002\u58ee\u5065\u306a\u82e5\u8005\u3067\u3082\u3001\u4e0a\u8ff0\u306e\u75c7\u72b6\u304c\u3042\u308c\u3070\u3001\u307e\u305a\u305d\u306e\u75c7\u72b6\u3092\u6cbb\u3059\u85ac\u3092\u6295\u4e0e\u3057\u3001\u8af8\u75c7\u72b6\u304c\u6d88\u9000\u3057\u3066\u304b\u3089\u9ebb\u6cb8\u6e6f\u3092\u6295\u4e0e\u3059\u308b\u5834\u5408\u306f\u554f\u984c\u3042\u308a\u307e\u305b\u3093\u3002\u4eee\u306b\u3053\u306e\u75c7\u72b6\u304c\u306a\u304f\u3066\u3082\u3001\u4e94\uff5e\u516d\u65e5\u524d\u3088\u308a\u534a\u7009\u3001\u3042\u308b\u3044\u306f\u832f\u82d3\u98f2\u306a\u3069\u3092\u6295\u4e0e\u3057\u3001\u5f8c\u65e5\u9ebb\u6cb8\u6e6f\u3092\u6295\u4e0e\u3059\u308c\u3070\u3001\u80f8\u306e\u3064\u304b\u3048\u3084\u5614\u5410\u306e\u75c7\u72b6\u306f\u3042\u308a\u307e\u305b\u3093\u3002\u4e16\u9593\u4e00\u822c\u306e\u533b\u8005\u306e\u4e2d\u306b\u306f\u9ebb\u6cb8\u6563\u3092\u6563\u5264\u306b\u3057\u3066\u6295\u4e0e\u3057\u3066\u3044\u308b\u4eba\u304c\u3044\u307e\u3059\u3002\u6563\u5264\u306f\u9ebb\u9154\u306e\u305f\u3081\u306b\u306f\u826f\u3044\u3068\u3044\u308f\u308c\u3066\u3044\u307e\u3059\u304c\u3001\u4eba\u306b\u3088\u3063\u3066\u306f\u6d88\u5316\u7ba1\u306b\u505c\u6ede\u3057\u3001\u80f8\u3084\u3051\u3084\u5614\u5410\u3092\u767a\u751f\u3059\u308b\u5834\u5408\u304c\u3042\u308b\u306e\u3067\u3001\u5148\u751f\u306f\u5e38\u306b\u714e\u3058\u305f\u9ebb\u6cb8\u6e6f\u3092\u6295\u4e0e\u3057\u3066\u3044\u307e\u3057\u305f\u3002<\/p>\n<div class=\"clearfix divider\"><\/div>\n<p>(2) if a patient does not readily regain strength after a hemorrhagic event, or is feeling an epigastric fullness, coughing and\/or expelling sputum, and experiences shortness of breath upon exertion, (3) if a patient feels strong palpitation, frequently feels fullness in stomach, and frequently yawns, experiences a heartburn, or vomits. The patient should be assessed carefully for the presence of any of the above three conditions, and then Mafutsuto can be used. If any of those conditions is present, the patient should receive a treatment for it first, and Mafutsuto can be administered upon its resolution. Even if a patient is young and naturally sturdy, the presence of above-mentioned conditions merits a prior treatment. Furthermore, even if a patient does not have those conditions, it is recommended to give Hannsya or Fureiin (they are both oriental medicines) 5 to 6 days prior to administration of Mafutsuto, to prevent the patient from feeling stomachfullness or vomiting when under anesthesia.<br \/>\nThere are many docters who use Mafutsusan, an anesthetic in a powder form, boiled extract of which is Mafutsuto (san in Mafutsusan means \u201cpowder\u201d, whereas to in Mafutsuto means \u201csoup\u201d). The powder form is said to be stronger than the extracted form, but the powder form tends to remain longer in the intestinal tract and might cause heartburn and vomiting in some patients. Therefore, our teacher always uses the extracted form.<\/p>\n<div class=\"clearfix divider\"><\/div>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-medium wp-image-187\" src=\"http:\/\/www.m.ehime-u.ac.jp\/school\/anes\/anes_wp\/wp-content\/uploads\/2016\/03\/mafutu21-204x300.jpg\" alt=\"mafutu21\" width=\"204\" height=\"300\" \/>\u9ebb\u6cb8\u6e6f\u6295\u4e0e\u5f8c\u306e\u4e09\u8a3a\u3068\u306f\u3001\u9ebb\u9154\u304c\u52b9\u3044\u3066\u3044\u308b\u304b\u3069\u3046\u304b\u3092\u77e5\u308b\u4e09\u3064\u306e\u8a3a\u5bdf\u6cd5\u3067\u3059\u3002\u4e00\u306b\u3001\u9ebb\u6cb8\u6563\u3092\u4e0e\u3048\u30661\u6642\u9593\u307b\u3069\u3067\u3001\u3057\u304d\u308a\u306b\u5c3f\u610f\u3092\u50ac\u3057\u3001\u8108\u306f\u300c\u6d6e\u300d\u3067\u300c\u6570\u300d\u3068\u306a\u308b\u3002\u4e8c\u306b\u3001\u5de8\u91cc\u306e\u52d5\u304d\u304c\u5f37\u304f\u306a\u308a\u3001\u5507\u3084\u820c\u304c\u4e7e\u71e5\u3059\u308b\u3002\u4e09\u306b\u3001\u9854\u306f\u9152\u306b\u9154\u3063\u305f\u3088\u3046\u306b\u306a\u308a\u3001\u77b3\u5b54\u306f\u6563\u5927\u3059\u308b\u3002\u3053\u306e\u4e09\u3064\u306e\u72b6\u614b\u3092\u78ba\u8a8d\u3059\u308c\u3070\u9ebb\u9154\u3055\u308c\u305f\u3068\u8a8d\u8b58\u3057\u3066\u3082\u3088\u3044\u3002\u3053\u306e\u3068\u304d\u306b\u306f\u3001\u3046\u308f\u8a00\u3092\u8a00\u3063\u305f\u308a\u3001\u773c\u306e\u7126\u70b9\u304c\u5408\u308f\u305a\u3001\u5b99\u3092\u307f\u305f\u308a\u3001\u76ee\u306b\u6620\u308b\u3082\u306e\u304c\u52d5\u304f\u3088\u3046\u306b\u898b\u3048\u3001\u666e\u901a\u306e\u72b6\u614b\u3067\u306f\u306a\u3044\u3088\u3046\u306b\u898b\u3048\u308b\u3002\u304a\u3057\u306a\u3079\u3066\u3001\u9ebb\u6cb8\u6e6f\u3092\u6295\u4e0e\u3057\u3088\u3046\u3068\u3059\u308b\u6642\u306f\u3001\u8108\u3001\u77b3\u5b54\u3001\u5fc3\u97f3\u3092\u5341\u5206\u8a3a\u5bdf\u3059\u3079\u304d\u3067\u3059\u3002\u4eba\u306b\u3088\u3063\u3066\u306f\u5e38\u306b\u8108\u304c\u901f\u3044\u8005\u3001\u77b3\u5b54\u304c\u5c0f\u3055\u3044\u8005\u304c\u3044\u307e\u3059\u304c\u3001\u8853\u524d\u304b\u3089\u3088\u304f\u8a3a\u3066\u3044\u308c\u3070\u3001\u9ebb\u6cb8\u6e6f\u6295\u4e0e\u5f8c\u306e\u4e09\u8a3a\u306b\u304a\u3044\u3066\u8ff7\u3046\u3053\u3068\u306f\u3042\u308a\u307e\u305b\u3093\u3002<\/p>\n<div class=\"clearfix divider\"><\/div>\n<p>Three post-administration diagnoses are made while the anesthesia is induced, in order to assess whether the patient is adequately anesthesized. Three conditions indicate the adequacy of anesthesia: (1) if the patient experiences urinary urgencies, and exhibits the pulse that is \u201cfloating\u201d (palpable upon light compression of the artery, but not upon strong compression) and \u201cnumbered\u201d (tachycardia), (2) if the patient\u2019s apex beat becomes stronger, and the skin and the lips lose their turgor, (3) if the patient exhibits flushing, as if under alcohol intoxication, and pupil dilation. If those signs are observed at about one hour after administration of Mafutsuto, you may assume that the patient is adequately anesthesized. At this point, the patient appears to be in an abnormal state, and may exhibit signs of confusion, such as making incoherent speeches or staring blankly at nothing.<br \/>\nWhen the patient is under Mafutsuto, it is important to monitor changes in the pulse, the pupil size and the heart sounds closely. Some people naturally have fast heart rate or small pupil, but examining the patient carefully prior to anesthesizing helps one to monitor and assess those changes accurately. After being given Mafutsuto, the patient should be laid down in an isolated room with a quiet environment.<\/p>\n<div class=\"clearfix divider\"><\/div>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignright size-medium wp-image-188\" src=\"http:\/\/www.m.ehime-u.ac.jp\/school\/anes\/anes_wp\/wp-content\/uploads\/2016\/03\/mafutu22-199x300.jpg\" alt=\"mafutu22\" width=\"199\" height=\"300\" \/>\u9ebb\u6cb8\u6e6f\u3092\u6295\u4e0e\u3057\u305f\u5f8c\u306f\u3001\u60a3\u8005\u3092\u500b\u5ba4\u306b\u5bdd\u304b\u305b\u3001\u9759\u304b\u306a\u74b0\u5883\u3092\u6574\u3048\u307e\u3059\u3002\u7720\u308b\u8005\u3082\u3042\u308c\u3070\u3001\u4e00\u7761\u3082\u3057\u306a\u3044\u8005\u3082\u3042\u308a\u307e\u3059\u3002\u307e\u305f\u3001\u3057\u3070\u3089\u304f\u3001\u3046\u308f\u8a00\u3092\u8a00\u3046\u8005\u3082\u3044\u307e\u3059\u3002\u3044\u305a\u308c\u3082\u5f90\u3005\u306b\u9ebb\u9154\u72b6\u614b\u306b\u306a\u308a\u307e\u3059\u3002\u5927\u4eba\u3001\u5c0f\u5150\u3067\u306f\u9ebb\u6cb8\u6e6f\u306e\u6295\u4e0e\u91cf\u306b\u5dee\u304c\u3042\u308a\u307e\u3059\u3002\uff15\u3001\uff16\uff5e\uff11\uff10\u6b73\u307e\u3067\u306f\u3001\u5927\u4eba\u306e\u56db\u5206\u306e\u4e00\uff5e\u534a\u5264\u3001\uff11\uff10\uff5e\uff11\uff15\uff0c\uff16\u6b73\u306f\u534a\u5264\uff5e\u4e03\u5206\u5264\u3001\u305d\u308c\u4ee5\u4e0a\u306e\u5927\u4eba\u306f\u5168\u91cf\u3001\u3064\u307e\u308a\u4e8c\u9322\u516b\u5206\uff08\u7d04\uff11\uff10g\uff09\u3092\u714e\u3058\u3066\u670d\u7528\u3055\u305b\u307e\u3059\u3002\u3057\u304b\u3057\u3001\u5927\u4eba\u3001\u5c0f\u5150\u306b\u9650\u3089\u305a\u3001\u4f53\u8cea\u306b\u3088\u3063\u3066\u306f\u9ebb\u6cb8\u6e6f\u306e\u52b9\u304d\u306e\u60aa\u3044\u8005\u3082\u3044\u307e\u3059\u3002\u5927\u4eba\u3067\u3001\u525b\u6bc5\u306a\u6027\u683c\u3086\u3048\u306b\u9ebb\u6cb8\u6e6f\u304c\u52b9\u304b\u306a\u3044\u8005\u3082\u3044\u308b\u304c\u3001\u5c0f\u5150\u306b\u304a\u3044\u3066\u3082\u9ebb\u6cb8\u6e6f\u304c\u52b9\u304d\u306b\u304f\u3044\u8005\u3082\u3044\u307e\u3059\u3002\u6628\u5e74\u3001\u5b87\u548c\u5cf6\u306e\u786f\u6751\u306e\u5b50\u4f9b\u304c\u706b\u50b7\u3092\u8ca0\u3044\u3001\u307b\u307b\u306e\u4e0b\u5074\u304c\u7652\u7740\u3057\u305f\u305f\u3081\u3001\u5148\u751f\u306b\u6cbb\u7642\u306e\u4f9d\u983c\u304c\u6765\u307e\u3057\u305f\u3002\u3053\u306e\u6642\u3001\u9ebb\u6cb8\u6e6f\u534a\u5264\u3092\u6295\u4e0e\u3057\u307e\u3057\u305f\u304c\u3001<\/p>\n<div class=\"clearfix divider\"><\/div>\n<p>Some patients fall asleep, some don\u2019t, and some may speak nonsensically. In all cases, the patients gradually go into anesthesia. There is a difference in doses of Mafutsuto for adults and children. The full dose for adults is approximately 10g. For children aged 5 ~ 10, the dose should be about one quarter of that, and for children aged 10~16, the dose should be about a half of that. In all age groups, however, there are individuals who are poorly responsive to Mafutsuto.<br \/>\nFor adults, unresponsiveness to Mafutsuto is often attributed to their personal characters, but there are young children who are unresponsive to Mafutsuto. Last year, our teacher had a request for an operation on an adhesion scar on the lower cheek of a small boy, which resulted from a burn. The patient came back in to our hospital days later, and this time our teacher used one-and-a-half dose of alcohol-extracted Mafutsuto, and this finally put him into adequate anesthesia. Unresponsiveness to Mafutsuto is a diathesis that can be seen even in small children like this patient, who was a 7-year-old, and makes the patient management very difficult. It is important to understand that a patient\u2019s responsiveness, or unresponsiveness, to Mafutsuto is a diathesis stemming from his\/her physical property, and not related to his\/her personality or temperament.<br \/>\nGenerally speaking, the factors influencing the outcome of surgical operations are the daylight length, a number of locations to be operated, and the complexities of the operative procedures. Even in the case of a major operation, the operation should be started in the afternoon, and the duration should never exceed two hours. Therefore, when a patient is allowed to have breakfast on the day of the surgery, he\/she should consume only 2\/3 of the usual amount, and administration of Mafutsuto should take place during the morning hours. If a patient does not go into anesthesia even at 3 hours after administration, he\/she should be considered unresponsive to Mafutsuto. Such patient should have their operation rescheduled for the other date, and be given alcohol-extracted Mafutsuto the next time. For patients unresponsive to Mafutsuto, giving them Mafutusan (powder form) with alcohol is another way to achieve anesthesia. However, as described earlier, the powder form increases the risk of vomiting, and our teacher suggests the use of it be avoided.<br \/>\nAccording to our teacher, in case of tumor resection, after the resection is completed, the remaining task is to give Kinso treatment (Kinso treatment is a treatment performed on a wound to minimize the risk of wound complications, such as wound infection, and consists of procedures such as bleed control, wound site disinfection, and suturing). At the beginning of the surgery, the patient should be blindfolded. This is because some patients fear they might feel pain even under anesthesia. Such patients may become nervous or even panic when they see surgical tools. Some even struggle with an enormous force. Therefore, the patients need to be restrained securely in order to perform a meticulous operation.<br \/>\nThe duration of time for which a patient remains in an adequate anesthesia varies among patients, and some patients come out of anesthesia faster than the others. Therefore, it is recommended an operation be started at the point the patient is in near (80~90%) full anesthesia. This allows the patient to go into full anesthesia while the surgery is proceeding. Knowing when is the right timing to start a surgery is very crucial to safely carry out an operation. There\u2019s a very telling case of that, in which a doctor performed a surgery on a tumor of the breast with an inadequate dose of Mafutsuto. But the doctor regarded her complaints as mere nonsense of a deliriant under anesthesia, and kept proceeding with the surgery. By the time the tumor was resected, the unfortunate patient had passed away. This is a horrible mistake stemming from diagnostic incompetence of the doctor, and he, in my opinion, is a despisable doctor, a grave sinner of the medical society.<br \/>\nThere are doctors, who use Mafutsuto yet don\u2019t adequately perform the peri-anesthesia diagnoses, and thus make improper patient management. This has spawned a belief among some lay folks that the patients who receive Mafutsuto for their operation can\u2019t live any longer than 3 years. Some doctors, who are incompetent in using Mafutsuto, argue for this belief, in an attempt not to recognize, or even to hide, their ignorance and incompetence. Such doctors deserve an utter contempt.<br \/>\nPatients under anesthesia often complain of agony, or some even make speeches as if in a fully awake state. Some patients under anesthesia may exhibit a seemingly unimpaired cognitive function, and may be able to accurately describe the objects they are presented to. Even in such patients, adequacy of anesthesia can be assessed reliably by performing the three post-administration diagnoses.<br \/>\nAfter the completion of the operation, the patient should be given a cooled cup of tea by little amounts, followed by Ryokutouto (green-pea soup, believed to have antipyretic and diuretic effect), also cooled and by little amounts. They are designated as the standard emergence-medications. The emergence time varies among patients. There are few patients who only take 4~6 hours to emerge from anesthesia, but average time is between 8~10 hours. On the following day, a patient in an apparent fully awake state may complain about a vision problem, in which stationary objects appear to be moving. This is because his\/her pupils are still dilated, and this does not warrant any concerns. Upon returning of the pupils to their normal sizes, the vision problem shall resolve.
